Our nurses create a comprehensive plan of care based on your impairments and collaborate with your physician to provide care. We focus on interventions that improve safety in the home and reduce the risk of hospitalization.
Our psychiatric nursing staff specializes in evaluating and treating psychiatric conditions. They create a plan of care based on assessments and patient’s needs. Treatment is a collaborative effort between the patient, family, and physician. Each patient is unique and our nurses create individualized programs which provide optimal outcomes.
The social workers at Peoples are dedicated to providing psychosocial support to promote positive health and well-being. Our medical social workers are here to help you deal with your diagnosis, health condition or impairments related to aging. They are a resource in helping you maximize your family, caregiver and community support systems and will assist you with long term planning and counseling.
We work with you to create an individualized program which helps you reach maximum potential. Treatment is focused on improving independence, improving mobility, reducing fall risks and reducing hospitalizations. Your physical therapist will perform an assessment and create a plan of care based on your personal goals and impairments. Treatment may include therapeutic exercise, balance training, vestibular training, ROM activities, pain modalities and gait training.
Our occupational therapists help improve your ability to perform activities of daily living such as bathing, dressing, eating and bowel/bladder management. They collaborate with other clinicians in the home to optimize your treatment. Treatment may include therapeutic exercise, transfers, bed mobility, bathing activities, dressing activities and adaptive equipment recommendation.
Speech therapists assist with deficits involving language, memory and swallowing. Our clinicians are experts at working with patients through a wide range of conditions including Alzheimer’s Disease, Parkinson’s Disease and CVA’s. Treatment may include oral exercises, memory activities, compensatory strategies and breathing techniques.
Our home health aides assist with activities of daily living and provide personal assistance in collaboration with nursing and therapy. Services may include activities such as meal preparation and assistance with bathing, grooming or dressing.
